Internal Medicine

The following branches of internal medicine have been established in the department.

Endocrinology and Metabolism Diseases: Diagnosis and treatment of thyroid gland, pituitary, adrenal gland diseases, diabetes (diabetes) and obesity (obesity), osteoporosis (bone loss) are provided. All kinds of hormone tests are performed.

Gastroenterology: Diagnosis and treatment of dieases of the liver, gall bladder, pancreas, stomach, esophagus and gastrointestinal system are provided. Gastroscopy, colonoscopy and biliary tract endoscopy (ERCP) are performed. Serum liver function tests, pHmeter of esophagus and liver biopsy are routinely done.

Hematology: Diagnosis and treatment of bone marrow diseases (anemia, acute and chronic leukemias) are provided. Serum blood tests and bone marrow aspiration are performed.

Nephrology: Diagnosis and treatment of kidney diseases, chronic kidney disease, urinary system diseases, high blood pressure and renal replacement therapies are provided. Serum renal function tests and urine analysis, contrast-enhanced kidney film, ultrasonography, central vascular access and kidney biopsy are performed. Experienced doctors and nurses serve in the hemodialysis unit. Continuous ambulatory peritoneal dialysis is provided.

Rheumatology: Diagnosis and treatment of rheumatoid arthritis and connective tissue diseases, vasculitis, osteoarthrosis , osteoporosis and gout are provided .

Medical Oncology: Diagnosis and treatment of cancer are provided . Chemotherapy for certain type of cancers are given to patients in chemotherapy unit.



Endokrinoloji Bilim Dalı

The endocrine system is a system of endocrine glands. The major endocrine glands are hypothalamus, pituitary and pineal glands at the brain, thyroid and parathyroid glands at the neck, and pancreas, adrenal glands and gonads (ovaries for women and testicles for men) at the abdomen.

Endocrine glands secrete what we call hormones and release them to the bloodstream. Hormones are responsible for the growth, development, reproduction, metabolism, and various balances in the body.

Endocrinology is a department that examines the hormones secreted from the endocrine glands, the effects of these hormones in our body and metabolic diseases. Endocrinology and metabolic diseases is a sub-specialty of the department of internal medicine. Endocrine diseases are the most common diseases in the society. Diabetes mellitus, goiter, hypothyroidism, hyperthyroidism, hair growth, infertility, menstrual disorders, obesity, osteoporosis, acromegaly, growth retardation, Cushing's syndrome are some of them.

Services Provided

4 endocrinology outpatient clinics, 2 diabetes, 1 obesity, 1 specific diseases, 2 nutrition, 2 diabetic foot outpatient clinics and 1 insoles workshop are provided in our diabetes and obesity center with a 400m2 closed area. In the specific diseases outpatient clinic; diagnosis, treatment and follow-up of thyroid cancer (on Mondays), pituitary and adrenal gland diseases (on Tuesdays), individuals with gender dysphoria (on Wednesdays) and metabolic bone diseases are carried out. Diagnosis, follow-up and treatment of patients hospitalized in the 17-bed endocrinology ward are fulfilled by specialist doctors and lecturers.

All endocrinological dynamic tests for diagnostic purposes can be applied to the patients in our test room belonging to our outpatient clinic or in our ward. Oral glucose tolerance test (OGTT) is performed every day in our outpatient clinic, and there is a separate test room where patients are under surveillance and waiting during the test.

In our clinic, thyroid ultrasound, ultrasound-guided fine needle aspiration biopsy, and thyroglobulin and parathormone washouts are performed by a specialist doctor or lecturer with an ultrasound guidance.

The education of diabetes and/or insulin therapy is given by 2 diabetes nurses in the outpatient clinic or by our ward nurse at the bedside to all diabetic patients who are followed up as outpatient or inpatient by our department. Apart from one-on-one patient education, diabetes education groups are gathered every afternoon during the week. Patient follow-up and insulin titration are performed by our nurses through telephone calls. Additionally, a 2-week "diabetes patient school" is held 4 times a year. In addition to endocrinology specialists, nurse educators, a dietitian, a podiatrist and a psychologist also attend the trainings.

Diagnosis and treatment of diabetes, and follow-up of diabetic complications are provided in 2 diabetes outpatient clinics every day. Insulin pump is applied to the required patients as outpatient or inpatient, and pump education is given gradually. Continuous subcutaneous blood glucose monitoring application and its education is also performed in our outpatient clinic. In our clinic, diabetic foot care, follow-up and treatment are carried out by a specialist podiatrist, and proper insoles are produced at our workshop following a detailed gait analysis.

Multidisciplinary team meetings are performed regularly in our clinic for the evaluation of controversial patients:

Thyroid, parathyroid and adrenal gland diseases meeting, with the contributions of the departments of General Surgery and Nuclear Medicine,

Pituitary diseases meeting, with the contributions of Neurosurgery Department,

Obesity meeting, with the contributions of the departments of General Surgery, Psychiatry and Anesthesiology, and a dietician

Gender Dysphoria meeting, with the contributions of Psychiatry, Gynecology and Obstetrics, Urology, Forensic Medicine, ENT/Head and Neck Surgery Departments.

History

Our department, which was founded by Prof. Dr. Abdullah Hacıhanefioğlu in 2000, has trained nine hematology specialists to date (as of March 2022, there are four subspecialists). Our clinic alone has met the heavy patient load of the Western Black Sea region for a long time. Our Hematology Department contributed to the establishment of the blood center with in the Faculty of Medicine. Currently, one of our faculty members of the Department of Hematology,Prof. Dr. Özgür Mehtap Works as director of The Blood TransfusionCenter. Bone marrow transplantation unit was opened in 2016 and autologous stem cell transplantation application was started. So far (as of March 2022), 103 patients have under go neautologous stem cell transplantation. Ourhospital has 32 beds in theDepartment of Hematology. Diagnosis and treatment of diseases related to anemia, hematological malignancies, bleeding disorders are performed. Our center, which also participates in multi-center studies as research, continues to providesub-branch training and aims to improve the training of sub-branch specialists in cooperation with hematology associations.

Polyclinics:

One or two specialist physicians serve under the supervision of 3 or 4 assistant physicians and a faculty member, although then umbervaries. An average of 70 patients are seen daily. Our polyclinic Works with an appointment system, except for emergency patients.

In our hematology service, one or two specialist physicians, assistants and alternately a faculty member diagnose and treatin patients.

There is an Apheresisunit in our Hematology Department, both stem cell collection and the rapeuticapheresis procedures are performed.

Nefroloji Bilim Dalı

The Department of Nephrology was established in 2000. There are four senior nephrologists, three nephrology trainees, one nephrologist, 12 internal medicine fellows and team of health professionals in the department. Nephrology department provides diagnosis and treatment of congenital kidney diseases, hypertension, chronic kidney diseases and dialysis (hemodialysis or peritoneal dialysis) . In addition to outpatient care, patients receive inpatient diagnosis and treatment in the 24-bed nephrology service. Special outpatient clinics are provided for kidney diseases, glomerulonephritis, autosomal dominant polycystic kidney disease and kidney transplant patients. Special follow-up and treatment are provided to prevent the progression of the disease in autosomal dominant polycystic kidney disease.

In the dialysis unit, hemodialysis treatment, vital plasmapheresis and other specific plasma exchange procedures are performed successfully. The peritoneal unit, where more than a hundred patients are followed, is one of the major centers in the region. Vascular access complications that occur frequently in hemodialysis patients are treated by an experienced team in the nephrology unit, and ultrasound- guided central vascular catheterization is achieved successfully.

In nephrology department there are blood pressure monitoring devices that can monitor blood pressure for 24 hours, an ultrasonography device used in kidney biopsies, 24 hemodialysis machines used for hemodialysis treatments, and 2 plasmapheresis devices used for plasma treatments. Kidney biopsy, which has diagnostic significance in kidney diseases, is performed and evaluated by pathologists specialized in renal pathology. All diseases are successfully diagnosed and treated by a team of experts in the department of nephrology.

The Department of Internal Diseases, Division of Rheumatology was established in 2001. In addition to educational activities and clinical research, it provides outpatient and inpatient care.

Diagnosis and treatment of inflammatory joint rheumatism (rheumatoid arthritis), inflammatory rheumatism of the spine (ankylosing spondylitis), soft tissue rheumatism (fibromyalgia, tendinitis, bursitis, etc.), systemic inflammatory rheumatic diseases of muscles, vessels, and connective tissue (myositis, vasculitis, systemic lupus erythematosus, systemic sclerosis), osteoarthrosis (calcification), gout, Familial Mediterranean Fever, and Behçet's Disease are performed.

Diseases that cause pain, sometimes deformity, and disability as a result of impairment in the muscles, ligaments bones, and joints that allow movement are called rheumatism. Some inflammatory rheumatic diseases can affect our internal organs (lung, kidney, brain, etc.). Rheumatism is not a single disease, nearly 200 diseases are included in this classification.

Rheumatic diseases are more common in women and their frequency increases with age. However, there are also rheumatic diseases that can be seen more in men (gout, ankylosing spondylitis), young adults (systemic lupus erythematosus), or children.

The exact causes of most rheumatic diseases are unknown. They are not usually contagious. Genetic predisposition has been found to be responsible for some rheumatic diseases.

In order to make the most appropriate treatment for rheumatic diseases, it is necessary to diagnose the disease correctly in the first stage. Sometimes it may be difficult to make a definitive diagnosis in the early period and the patient may need to be followed up for a while. Symptoms of the disease may change over time. Most of the rheumatic diseases are long-term, can last for years, and are called "chronic". In this case, it should be noted that the treatment will take a long time and patients should be under the supervision of a doctor. Even if the treatment does not completely eliminate the disease, it allows the patients to continue their lives painlessly and comfortably and prevents the development of permanent skeleton or organ damage that may occur due to the disease.

Currently, our department provides service with 3 lecturers, 2 rheumatology specialists, and 5 rheumatology fellows. Approximately 100 patients are served daily in ambulatory service. We have an inpatient service with a capacity of 12 patients, and approximately 20 patients are treated daily in the outpatient infusion unit.

Tıbbi Onkoloji Bilim Dalı

The Medical Oncology Department of Kocaeli University provides diagnosis, treatment and follow-up of all cancer types originating from any tissue or organ. We apply therapies including chemotherapy, hormonal treatments, targeted therapies and imnunotherapies. All these treatment plans for cancer patients are developed through a multi-disciplinary approach in cooperation with the departments of Radiation Oncology, General surgery, Urology, Radiology, Nuclear Medicine, Pulmonary diseases, Thoracic Surgery and Gynecology and Obstretrics.

Services

In our department, new patient admission and follow-up and treatment practices are carried out in 3 outpatient clinics. About 150 outpatients are treated daily. In the outpatient chemotherapy unit, chemotherapy treatments are carried out with a total of 45 seats and approximately 80 patients per day. In our oncology service, follow-up, treatment and care services are provided with 34 beds.

Educational activities

The education and training activities of the Faculty of Medicine, Term 4 and 6, determined by the Department of Internal Diseases, are carried out. At the same time, training activities, including weekly article and seminar hours, are carried out with Internal Medicine assistants and Oncology fellows.

Researches

Scientific and experimental studies are carried out by Medical Oncology faculty members together with Internal Medicine assistants and Oncology fellows. In addition, our faculty members carry out international Clinical Phase studies together with the Clinical Research Center within our hospital.
