Studies of Kocaeli University Medical Faculty Department of Physical Medicine and Rehabilitation

Kocaeli University Department of Physical Medicine and Rehabilitation gives health services both in Kocaeli University Hospital and in İREM (İzmit Rehabilitation Center) on following subjects:


Neurological Rehabilitation (Rehabilitation of Neurological Diseases)

In our department we rehabilitate all neurological diseases including spinal cord injury, cerebrovascular accident related hemiplegia (stroke), traumatic brain injury, multiple sclerosis, Parkinson’s disease. We also focus on swallowing problems, neurogenic bladder and bowel, sexual dysfunctions that develop in these neurological diseases. Regarding these diseases, new treatment modalities such as “constraint induced movement therapy” and “neurofeedback” are being used firstly in this clinic in Turkey.


Diagnosis, Treatment and Rehabilitation of Rheumatologic Diseases and Osteoporosis

We diagnose, treat and rehabilitate rheumatologic diseases, arthritis (joint inflammation), back and cervical pain in our outpatient clinics. Furthermore, we deal with osteoporosis in special days of outpatient services. We make patient support groups and teach, rehabilitate, and give exercises regarding ankylosing spondylitis, rheumatoid arthritis and osteoporosis patients. Physical therapy modalities, injection modalities, magnetotherapy, and manipulation therapies are also applied to our patients.


Orthopedic Rehabilitation

We rehabilitate all orthopedic conditions including bone fractures, joint prosthesis applications, anterior cruciate ligament injuries and other sport injuries, and patients exposed to spinal and hand surgeries.


Pediatric Rehabilitation

Commonly we rehabilitate children with cerebral palsy, spina bifida and brachial plexus injury. Cerebral palsy and spina bifida patients are scrutinized in specified outpatient clinics. We have special “Pediatric Rehabilitation Unit” that personal and group therapies applied in İzmit Rehabilitation Center (İREM). In pediatric rehabilitation studies we organize therapies based on “Dynamic System Model”. “Constraint Induced Movement Therapy” is also used when needed.

Children with swallowing problems are specifically examined and rehabilitation programmes are organized. And also all pediatric patients are evaluated by a special educator and introduced to personal and/or group education programmes when it is necessary.


Rehabilitation of Swallowing Problems

The first unit of our country in this field gives diagnosis, treatment and rehabilitation services related to swallowing problems depending on neurological diseases and mechanical causes. After the clinical evaluation of patients, videofluoroscopic examinations are used when needed, and physical therapy and rehabilitation modalities are applied. Although lots of patients having swallowing problems do not have any symptoms, these problems can lead to complications (even mortality). Although the results of the treatments are successful, unfortunately not enough attention is given to this area in our country.


Rehabilitation of Neurogenic Bladder and Bowel dysfunctions

Another specialty of our department is the rehabilitation studies of neurogenic bladder and bowel dysfunction resulted from neurological diseases. In our studies we aim discontinuation of indwelling urinary catheterization and use clean intermittent catheterization and/or spontaneous micturition. All patients with neurogenic bladder are evaluated by manual cystometric studies at bedside and when needed we plan videourodinamical studies done by the Urology Department.


Hand Rehabilitation

After surgical repairs of patients with tendon-nerve injured hands we immediately start rehabilitation programmes to have maximum healing results. Our doctors strictly examine hand rehabilitation patients and use Washington, modified Duran, early passive mobilization or immobilization protocols accordingly with physiotherapy and occupational therapies.


Cardiac/Pulmonary Rehabilitation

Patients with coronary heart disease and congestive heart failure have decreased aerobic capacity and limitations in daily living activities. Patients examined by the Cardiology Department and who have no contraindications to exercise are monitorized to apply cardiac rehabilitation programmes in our clinic.

Patients referred from the Pulmonary Medicine Department (mainly chronic obstructive pulmonary disease) are introduced to pulmonary rehabilitation programmes in “Pulmonary Rehabilitation Unit”

Since June 2003, constraint induced movement therapy has been used for hemiplegic and cerebral palsy patients.

*Constraint induced therapy: As the name indicates we constrain healthy upper extremity of a patient by glove and arm sling and apply therapy programme for 5-7 hours per day. Appropriate patients to this treatment will start to use their disabled hand in functional activities after having 2-3 weeks of therapy. This treatment is used both in adult and children hemiplegics successfully.

Physical therapy and rehabilitation techniques, and “Dynamic System Model” are used for cerebral palsy patients in İzmit Rehabilitation Center (İREM). Yahya Kaptan Occupational Therapy School is the first school in this field in Turkey, and studies related to pediatric rehabilitation has been continuing in occupational therapy school.

All pediatric patients are evaluated by a special educator and introduced to personal and/or group education programmes when it is necessary.
